2008 REGISTRATION

You can register for the upcoming Fall season 2 different ways:

· in person at one of the sign-up days, where you can:

-fill out a Registration Form

-fill out a Uniform Information Sheet

(samples will be available to size up kids)

-pay the registration fees

OR

· online at eayso.org where you are able to:

-fill out, print out and sign two copies of the Registration Form

-print and fill out a Uniform Information Sheet

(click here for sizing instructions)

-write a check for the Registration Fees

(there is no ability to pay fees online)

and get the above to the Region by either:

A) dropping them off at one of the Saturday Walk in Registration Sign-Up Days, 11am to 2pm:

Fees include: Uniform, Insurance, soccer ball, team pictures, and trophy

May 3 Fees $80

May 31 Fees $90

June 7 Fees $90

OR

B) mailing them to: PO Box 697, Rio Linda CA 95673

Fees:

April 1 - May 3rd $80

May 4th - June 7 $90

after June 7, are $100

Note: New players are required to show proof of age. For example: a child’s birth certificate or a medical

ID card with the child’s birth year and month. Please bring this information to the in person sign-ups

or include a photocopy with the signed forms & fees if you are mailing them in.
